B'nai Mitzvah Date Selection Form

Mazal tov! We are excited to begin planning your child's b'nai mitzvah with you.

Please submit this form to indicate your family's top three date preferences. Rabbi Ariella and I will review the dates and will contact you to confirm.

When you are exploring dates, please refer to the calendar on the CBI website to make sure there are no b'nai mItzvah already scheduled for the dates you are considering.

B'nai mitzvah dates must fall after the 13th Hebrew birthday for those who identify as boys, and after the 12th Hebrew birthday for those who identify as girls. Rabbi Ariella will check all date requests to make sure they meet these guidelines.

We are so excited to support you through the b'nai mitzvah process and to celebrate this very special milestone with you.

Each family must become a member of Congregation B'nai Israel in order to reserve a b'nai mitzvah date. This supports our staff in meeting the needs of your family throughout the process, and engages your family in the full breadth of CBI's community life.

If your family is not yet a member of CBI, you can become one easily by filling out our CBI Membership Form. We offer a Fair Shares dues model to accommodate each family's needs.
We know that each family is unique and approaches this milestone with different questions. Please share a bit about yourselves.
Feel free to share your childs’ unique strengths, interests, and challenges.
